Located at Tift Regional Medical Center, the Congestive Heart Failure (CHF) Clinic provides specialized care and education for patients diagnosed with
congestive heart failure (a condition in which the heart can’t pump enough blood to meet the body’s needs). In some cases, the heart can’t fill with enough blood and has trouble relaxing. Or, the heart can’t pump blood to the rest of the body with enough force. Some people have both problems. The goal is to help patients better understand their condition, manage their health, minimize the need for hospital readmission, and improve their quality of life through services such as close monitoring of symptoms and vitals, specialized care coordination, and extensive education.

The CHF Clinic team works closely with referring doctors to keep them fully apprised of patient progress. Improved quality of-life is the aim. Treatment may include lifestyle changes, medications, surgery or medical devices. The CHF Clinic team ensures patients are taking the right medications at the right doses, they understand dietary and fluid requirements, and recognize the signs and symptoms that may signal a worsening of their condition.

Signs and Symptoms of CHF include:
· Shortness of breath
· Swelling of legs
· Weight gain
· Fatigue
· Difficulty performing daily tasks
· Poor Sleep
· Dizziness

Services provided include:
· Close monitoring of symptoms and vitals
· Coordination of specialized medical and nursing care with cardiologists, cardiac rehabilitation other healthcare professionals
· Extensive education on medication, diet and lifestyle changes

A similar clinic is also provided for patients with COPD.
