Any health problem impacts your life, but a condition that has dealt you a limited-life expectancy is especially difficult.  You and your family are faced with numerous changes, challenges and questions. Now there is an answer, a helping hand to help guide you to living your life to the fullest.
 
The Transitions Program is a non-medical program that offers support, resource management and volunteer services to people with terminal illnesses.  It is designed to enhance the quality of life for patients and their families.
 
When someone is diagnosed with a serious illness, they will often need support and help finding answers while dealing with their illness.  The Transitions Program has been created to provide information and community resources for patients who are faced with the new and strenuous challenge of a life-threatening illness.
 
With Transitions, Tift Regional Medical Center will offer consistent, caring support for people struggling with all the aspects of their illness.  The Transitions staff will be there as a listening ear during the process of going to medical and treatment appointments.  Transitions can connect patients to various helpful resources in the community.  Patients often find it helpful for our volunteers to run errands, make periodic visits to give the caregiver a break or just provide companionship.
 
Who is eligible for Transitions?
Any person in the community who has been diagnosed with a terminal illness with a limited life expectancy. The typical Transitions patient may still be receiving aggressive curative treatment for his/her disease.
 
What is the cost to the patient/family?
Nothing.  The Transitions program is funded by community gifts and Tift Regional Medical Center.
 
How does someone become a Transitions patient?
It's easy!  Referrals may come from your physician, the discharging hospital, the patient, the patient's family or any other participant involved in the patient's care.  Just call the Transitions office to inquire: (229) 353-6373  or TRMC toll free: 1-800-648-1935.
