The CHF Clinic provides specialized care and education for patients diagnosed with congestive heart failure. The goal is to help patients better understand their condition, manage their health, and minimize the need for hospital readmission. Congestive Heart Failure (CHF) is a condition in which the heart can’t pump enough blood to meet the body’s needs. In some cases, the heart can’t fill with enough blood and has trouble relaxing. Or, the heart can’t pump blood to the rest of the body with enough force. Some people have both problems. For a patient to be enrolled in the clinic, a referral must be made by the patient’s physician.
The CHF Clinic team works closely with referring doctors to keep them fully apprised of patient progress. Improved quality of-life is the aim. Treatment may include lifestyle changes, medications, surgery or medical devices. The CHF Clinic team ensures patients are taking the right medications at the right doses, they understand dietary and fluid requirements, and recognize the signs and symptoms that may signal a worsening of their condition.
Signs and Symptoms of CHF include:
· Shortness of breath
· Swelling of legs
· Weight gain
· Difficulty performing daily tasks
· Poor Sleep
Services provided include:
· Close monitoring of symptoms and vitals
· Coordination of specialized medical and nursing care with cardiologists, cardiac rehabilitation other healthcare professionals
· Extensive education on medication, diet and lifestyle changes
