Weight Loss Clinic

Family nurse practitioner Todd McDonald offers a medical weight loss clinic at Sylvester Family Practice.  Whether you have tried to lose weight before, or this is your first diet, Todd will partner with you to shed those pounds and maintain a healthy weight.   Once Todd has completed a medical history including your body composition analysis, he will want to understand your goals.  Then, Todd will develop a customized plan to best suit your needs.

DOT Physicals

The Federal Motor Carrier Safety Administration (FMCSA) has named Dr. Wilder and Todd certified medical examiners to perform Department of Transportation (DOT) physicals for truck and bus drivers.  Both Dr. Wilder and Todd are members of the National Registry of Certified Medical Examiners (National Registry).  To become a certified medical examiner and be listed on the National Registry, healthcare professionals must complete training and testing on the FMCSA physical qualifications standards.  All interstate Commercial Motor Vehicle (CMV) drivers must have their physical examination performed by a certified medical examiner listed on the National Registry.  

Drivers with a Commercial Driver’s License (CDL) are required to have a DOT exam.  Also, drivers operating the following vehicles in interstate commerce need to have a DOT medical exam as well:

  • Vehicles with a gross vehicle weight (GVW) or gross vehicle weight rating (GVWR), or gross combination weight (GCW) or gross combination weight rating (GCWR) of 10,001 pounds or more; or
  • Vehicles designed to transport more than 15 people, or more than 8 people when there is direct compensation involved; or
  • Vehicles transporting hazardous materials that require the vehicle to be placarded.

About Our Providers

Louise Wilder, MD.  Dr. Wilder is certified by the American Board of Family Medicine.  After earning her medical degree from University of Alabama at Birmingham, Dr. Wilder completed an internship and residency in family practice at Florida Hospital in Orlando.  Before joining Sylvester Family Practice in 2006, Dr. Wilder practiced medicine for nine years in New Port Richey, Florida. 

Todd McDonald, FNP-BC.  Todd, a Worth County native, is a family nurse practitioner certified by the American Nurses Credentialing Center.  Todd earned his nurse practitioner degree from Troy University after receiving a bachelor’s degree in nursing from Valdosta State University and a master’s degree in health services administration from Columbia Southern University.  Todd served in various clinical roles in Southwest Georgia before joining the Sylvester Family Practice team in 2002.
