If you have access to an Adult Child’s patient portal, this access will expire on October 1, 2017 unless the individual authorizes your continued proxy access. This can be accomplished by completing, signing and returning the authorization form to Health Information Management by fax (229-353-7779), e-mail email@example.com, hand delivery (901 E. 18th St, Tifton, Georgia) or mail (Tift Regional Medical Center c/o Health Information Management, P.O. Box 747, Tifton, GA 31794.
An Adult Child is an individual who is over the age of eighteen (18) and less than twenty-one (21) years old.
If you are the court appointed legal guardian of an Adult Child, you can continue your access to the Adult Child’s patient portal by completing the authorization form and providing documentation of your guardianship.
If you are not the court appointed legal guardian and you have access to a special needs Adult Child’s patient portal, please message your Adult Child’s physician’s office. Request documentation in the medical record that the Adult Child is not competent to make their own decisions in order to continue your proxy access.
